Patient-Centered Medical Home Model
As a Patient-Centered Medical Home, OCHS provides health care that is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, OCHS works to ensure that our patients are fully informed partners in establishing care plans, with a focus on prevention, wellness, and quality health care.

Please note: all Outer Cape Health Services’ sites are designated as fragrance-free. For the health and safety of our staff, patients, and families, it is important that everyone come fragrance-free to our health centers. Many people in our community become ill when exposed to fragrances such as those contained in personal care and laundry products. Such products can contain unlisted toxins. While almost everyone may have some reaction to strong fragrances, some individuals have much more immediate reactions even to “lighter” fragrances – including migraines, blurred vision, muscle and joint pain, difficulty breathing, and even seizures. Thank you for understanding and recognizing this.
