Damian K.L. Archer, MD

Chief Executive Officer 

Dr. Damian Archer is a board-certified Family Medicine physician and a mission-driven leader committed to expanding equitable, high-quality care in underserved communities. As Chief Executive Officer of Outer Cape Health Services (OCHS), Dr. Archer guides a Federally Qualified Health Center that serves the ten outermost towns of the Lower and Outer Cape, delivering comprehensive primary care, behavioral health, recovery services, and community-based programs to residents and seasonal workers alike.

Dr. Archer brings more than a decade of clinical, academic, and equity-focused leadership to his role at OCHS. He maintains a clinical practice and holds several key appointments at Tufts University School of Medicine, where he serves as Assistant Dean for Multicultural Affairs, Clinical Assistant Professor of Family Medicine, and Director of the Sam W. Ho Health Justice Scholars Program. In these roles, he has shaped the next generation of physicians by advancing inclusive learning environments, mentoring emerging leaders, and elevating health justice across the curriculum.

A graduate of the University of the West Indies (MD) with residency training at the University of Wisconsin/Aurora Healthcare—where he served as Chief Resident—Dr. Archer has dedicated his career to strengthening community health systems, building culturally responsive clinical teams, and expanding access to compassionate, evidence-based care. Under his leadership, OCHS is embracing innovative models of team-based primary care, integrating medical and behavioral health services, and developing scalable programs that meet the evolving needs of the Cape’s diverse population. Dr. Archer’s vision centers on ensuring that every patient—regardless of background, identity, income, or geography—can rely on OCHS as their medical home.

Marie Andrine Constant, MD, MPH

Chief Medical Officer 

Dr. C. Marie-Andrine Constant is a board-certified family physician, public health practitioner, and values-driven clinical leader dedicated to advancing equitable, community-based care. As Chief Medical Officer of Outer Cape Health Services (OCHS), Dr. Constant oversees the clinical operations, quality of care, and medical leadership of a Federally Qualified Health Center serving the ten outermost towns of the Lower and Outer Cape. She leads OCHS’s efforts to deliver high-quality, team-based primary care while strengthening integrated behavioral health, chronic disease management, and population health programs across the organization.

Prior to becoming CMO, Dr. Constant served at OCHS as Chief Population Health Officer and Provincetown Site Medical Director, where she guided initiatives to optimize care delivery, expand value-based care performance, and support clinician education and development. She has extensive prior leadership experience, including serving as Associate Medical Director at a North Shore community health center and as a Supervising Physician at Mass General Brigham Urgent Care. Across these roles, she has championed innovative care models for patients with complex chronic conditions, co-occurring mental health needs, and health disparities rooted in social determinants of health.

Dr. Constant earned her medical degree from the State University of Haiti, School of Medicine and Pharmacy, and her Master of Public Health from the University of Massachusetts Amherst, where she focused on diabetes and health disparities. She completed her Family Medicine residency at St. Joseph’s Hospital Health Center in Syracuse, New York. Her professional training and lived experience inform her deep commitment to culturally grounded care, health equity, and strengthening access for marginalized and multilingual communities.

In addition to English, Dr. Constant is fluent in French, Haitian Creole, and Spanish, enhancing the organization’s ability to meet the needs of diverse populations. As CMO, she is leading OCHS toward a future of more accessible, coordinated, and responsive primary care—ensuring every patient can receive high-quality care close to home.

Kate Swanson, MBA

Chief Strategy Officer

Kate Swanson is a mission-driven healthcare strategist with over 20 years of experience guiding organizations through transformative change. As Chief Strategy Officer at Outer Cape Health Services (OCHS), she leads organizational planning, transformation initiatives, and cross-functional alignment to ensure the health center can meet the evolving needs of the ten communities it serves. Her work centers on translating OCHS’s mission and values into clear, actionable strategies that strengthen access, improve care delivery, and build sustainable models for the future.

Kate is recognized for her rare ability to operate seamlessly at both the visionary and operational levels—crafting strategies that honor community need while navigating the practical realities of implementation. Her career has been devoted to helping healthcare organizations adopt patient-centered, equitable, and financially resilient models of care. She approaches her work with a clear purpose: to simplify complexity, dismantle inequities, and ensure that every community has access to excellent, affordable care.

Before joining OCHS, Kate served as Executive Vice President and Chief Operating Officer of North Shore Community Health Center, where she led the organization’s value-based care transformation and redesigned operational structures to deliver more timely, personalized care. She also worked within Mass General Brigham supporting primary care practices across the system as they pursued NCQA Patient-Centered Medical Home accreditation, offering coaching, operational support, and training to strengthen team-based care.

Across her career, Kate has brought together diverse teams, broken down silos, and facilitated collaborative approaches that support meaningful growth, operational excellence, and superior patient experience. She is frequently invited to share her expertise at statewide healthcare forums, including as a featured speaker for the Massachusetts League of Community Health Centers’ Community Health Institute.

Kate holds an MBA with a concentration in Healthcare Policy and Management from Brandeis University’s Heller School, a BA from Stonehill College, and a certificate in Social Impact Management and Leadership from the Institute for Nonprofit Practice in collaboration with Tufts University’s Tisch College of Civic Life.
