Medicare patients over age 65 are the fastest-growing patient population at CHCs, nearly doubling over the last decade. Seniors increasingly select CHCs as their primary care provider of choice. However, the Medicare Prospective Payment System (PPS), which relies on fixed reimbursement costs, has not kept pace with increasing health care costs, especially for rural providers where the cost of care is even higher. 

Congressional action is necessary to close the gap and streamline comprehensive, effective primary care for seniors. In the House of Representatives, Reps. Michael Rulli (R-OH) and Debbie Dingell (D-MI) have introduced the CHC Reforms Enabling Better Access for Seniors and Elders (REBASE) Act of 2026 (H.R. 10398).

The CHC REBASE Act is a bipartisan, comprehensive Medicare solution that will: 

• Rebase the CHC Medicare PPS
• Update the “qualifying visit list” for reimbursable services at CHCs
• Provide payment parity for medical telehealth visits for CHCs and Rural Health Clinics
• Improve Medicare Advantage wrap payments for CHCs
