Outer Cape Health Services Names Dr. Matthew Libby Site Medical Director of New Harwich Port Health Center
HARWICH PORT, MA – 12/3/2018 – Outer Cape Health Services (OCHS) today announced Dr. Matthew Libby as the Site Medical Director of its new Harwich Port Community Health Center. In addition to his own clinical practice, Dr. Libby will oversee the provider clinical staff at the federally-qualified community health center designed to deliver team-based, patient-centered care. He reports to OCHS Chief Medical Officer, Dr. Andrew Jorgensen.
Dr. Libby, a Brewster resident, will also continue to serve as OCHS’ Clinical Director of Informatics and Director of Medical Education. He is board certified by the American Board of Family Medicine. Previously, Dr. Libby served as Site Medical Director of OCHS’ Wellfleet Health Center which will be temporarily closed for reconstruction and expansion over the next six months.
Most recently Dr. Libby completed a Primary Care Leadership fellowship with the prestigious Linde Family Institute for Primary Care at Beth Israel Deaconess Medical Center. The Linde Family Fellowship Program provides early and mid-career physician leaders with an opportunity to develop the expertise and skills needed to advance careers in primary care leadership, including practice management and innovation.
Dr. Libby received a B.S. in Physics from University of Massachusetts and later earned a Doctor of Osteopathic Medicine degree from the University of New England where he received the Sauter Award for Graduate with Distinction. He completed his Family Medicine residency at the Greater Lawrence Family Health Center. He began practicing primary care, urgent care, and outpatient addiction treatment at Outer Cape Health Services in Wellfleet in 2015.

The new 14,000+ square-foot center is located at 710 Route 28, Harwich Port. It features 16 patient exam rooms, 2 behavioral health consult rooms, medical "pod" team working spaces, a pharmacy opening to the public in January 2019, Quest lab services, a Women, Infants and Children (WIC) program, and a Healthy Connections office to assist patients with attaining and maintaining health insurance. Other features of the center include an ambulatory transfer area and a community wellness conference room. The organization’s patient call access center and administrative offices are located on the second floor of the same building.
Health center operating hours are 8 a.m. to 5 p.m. on Mondays, Wednesdays and Fridays and 8 a.m.to 7 p.m. on Tuesdays and Thursdays. Patient appointments can be scheduled by calling the OCHS Access Center at (508) 905-2888.
