NANCY L. GOLDEN, MD, Pediatrician
Nancy L. Golden, MD is Board certified. She obtained her MD from Tufts
University College of Medicine
in 1971 and completed her senior residency at Children’s Hospital,
Boston
in 1974. She was a
Fellow in Developmental Pediatrics and Developmental Disabilities at
North Carolina
Memorial
Hospital
in 1976. Dr. Golden
practiced pediatrics in the
Cleveland
area from 1976-1984, when she opened her own practice, Orleans Pediatrics Group, on Cape Cod. She joined Outer Cape Health in the summer of 2011,and has been on the active
staff at
Cape Cod
Hospital
(CCH) from 1984 to June
2011. Dr. Golden was Chief of Pediatrics at CCH from 1985-88. She has volunteered annually in medical relief
efforts for the country of
Haiti
for the last three years.

RICK LEVINE, MD, Primary Care
Dr. Levine is Board certified in Family Medicine. Dr. Levine graduated magna cum laude from Ursinus College in 1988, taught high school biology for three years prior to going to medical school and graduated from The Medical College of Pennsylvania in 1994. After completing a family medicine residency at the West Jersey Family Medicine Residency in 1997, Dr. Levine stayed on as faculty for 13 years. A New Jersey "Top Doc" in 2008, Dr. Levine is interested in all aspects of family medicine, from birth to grave, including well child visits, GLBT health, chronic disease management, and preventive medicine. Dr. Levine practices at OCHS in Wellfleet as well as Provincetown.

LAWRENCE G. MILLHOFER, MD, Primary Care
Dr. Millhofer is Board certified in Internal Medicine, and has been a practicing primary care internist for 32 years since 1978. Dr Millhofer especially finds that taking care of geriatric patients as well as patients with diabetes is very rewarding. Dr Millhofer works to ensure that all patients receive the highest standard of care. He is the chair of the Pharmacy and Therapeutics committee at OCHS. Dr Millhofer received his BA degree from Haverford College and his MD from SUNY at Buffalo where he was elected to the National Medical Honors Society. He completed his internship and residency in internal medicine with a focus on primary care at St. Mary's Hospital in San Francisco. Dr. Millhofer resides in Provincetown. |

LAWRENCE M. NEWMAN, MD, Primary Care
Dr. Newman is Board certified in Internal Medicine and has been practicing for over 32 years. Dr Newman is a graduate of Harvard University, obtained his Medical Degree from the University of Massachusetts Medical School in 1975, and performed his residency at Genesee Hospital, Rochester, NY, which he completed in 1978. |

Dr. Penn is
Board Certified in Internal Medicine. She received her BS in Biology

SUNY Buffalo in 1977. Dr. Penn completed her internship and residency in 1981 at

Hospital
and practiced Primary Care
medicine at the Massachusetts General Hospital (MGH) Charlestown Health Care
Center from 1981 until joining OCHS in 2012. She was the Associate Unit Chief of
the Adult Medicine Unit at the MGH Charlestown Health Care Center from
2007-2012. Dr. Penn received the Partners in Excellence Award in 2008 and the
Partners Health Care Serving the Underserved Award in 2007. Dr. Penn will begin
accepting new patients at our
Wellfleet
Health
Center
on January 27.

BARBARA K. PRAZAK, MD, Medical Director, Director of Clinical Quality
Dr. Prazak is Board certified in Internal Medicine. She is a 1976 magna cum laude graduate of Boston College with a BS degree in Biology, obtained her Medical Degree from the University of Massachusetts Medical School, Worcester in 1980, and performed her residency at Mount Auburn Hospital, Cambridge from 1981-1983. She has been a resident of Wellfleet and a physician at Outer Cape Health for over two decades. Dr. Prazak practices at OCHS in both Wellfleet and Provincetown. |

JENNIFER CONNORS, NP, Nurse Practitioner
Jennifer is a 1991 Bachelor of Science, Nursing graduate of the University of Rhode Island, and obtained a 1998 Master of Science in Nursing from The Mass General Hospital Institute of Health Professions. She has extensive experience in pre and post electrophysiology, including ablations, pre/post pacemaker (PM), implantable cardiac defibrillators (ICD) and cardioversions. Jennifer has facilitated management of outpatient care for electrophysiology patients including ordering and reviewing medication renewals, follow up laboratory, and cardiac testing/monitoring results. She is conversational in Spanish, and has been awarded the 1998 Brigham and Women's Hospital Nurse Recognition Award for Clinical Excellence. |

MEREDITH GOFF, Certified Nurse Midwife
Meredith has been a clinician, lecturer, independent and private practice midwife. She obtained her diploma in Nursing from New England deaconess Hospital in 1977, her Bachelor of Science in Nursing from the University of the State of New York in 1982, and Master of Science with Specialty in Nurse-Midwifery from Columbia University in 1983. She is Certified by the American College of Nurse Midwives, and has receive the regional Award for Excellence Nominee from the American College of Nurse Midwives, the Excellence in Teaching Award by the A.C.N.M. Foundation, the La Leche League Healthcare Provider Award, and the Midwifery Activist Award. She has made multiple presentations and has many peer reviewed publications, including the Journal of Midwifery and Women's Health. |

MELISSA HAACK, PA, Physician Assistant
Melissa is a Certified Physician Assistant, and is licensed in Iowa and Massachusetts. She graduated from Moorehead State University in 1996 with a Bachelor of Science degree in Speech/Language/Hearing Sciences, and the University of Iowa in 2008 with a Master of Science in Physician Assistant Studies. She has provided acute and chronic health care to patients with multiple, chronic medical conditions including type 1 and 2 diabetes, hypertension, COPD, asthma, congestive heart failure, cirrhosis and hepatitis C. She has managed anticoagulation therapy for patients taking Coumadin, and has performed punch biopsies, excision of skin lesions, toenail removal and suturing of simple wounds. She received the Rex Montgomery Physician Assistant Society Scholarship, and Chaired the Service Committee, University of Iowa Student Physician Assistant Society. |

Taylor Ricketson, NP, Pediatric Nurse Practitioner
Ms. Ricketson graduated summa cum laude with her BS degree in
Nursing from Boston College School of Nursing in 2010 and received a Master’s
Degree as a Pediatric Nurse Practitioner from Boston College Graduate School of
Nursing in 2011 with a GPA of 4.0. Her clinical experience during training
included work at Children’s Hospital, Chestnut Hill Pediatrics, and a year spent
under the guidance of Dr Nancy Golden at Orleans Pediatrics 2010-2011. Ms
Ricketson joined OCHS in October 2011. She recently moved to
Chatham
and enjoys seeing
pediatric patients of all ages for episodic and well child care.

SUSAN FLYNN RODERICK, CFNP, CPNP, BSN, Senior Nurse Practitioner, Director of the Teen Clinic
Susan obtained a BS in Nursing from the University of Rhode Island in 1971, her Pediatric NP in 1976 and Family NP Certificate from Boston's Northeastern University in1980. She was the Massachusetts League of Community Health Centers 2001 Employee of the Year, and has worked with many social service organizations, including Independence House, Provincetown Domestic Violence Intervention Office, Cape Cares, and Helping Our Women. She has provided health educational classes to students at both the middle and high school levels throughout the lower and outer Cape. Susan started the Teen Health Clinic in 1992 to teach and empower young adults to act responsibly, and has worked at OCHS for over 3 decades. |

ELLEN STEWART, PA-C, Physician Assistant
Is a Physician Assistant-Certified (PA-C) at Outer Cape Health Services, providing primary care to a complete spectrum of patients, and especially enjoys caring for families and children. Ms. Stewart graduated Cum Laude from Massachusetts College of Pharmacy and Health Sciences, and from the Rochester Institute of Technology with a Bachelor of Science. She began her career as an ophthalmic photographer, spurring
her interest in medicine. She is also an accomplished Kayak racer, and surfs the better Outer Cape breaks on a Long Board. |

JAMES ANDERSON, DPM, Podiatry
Dr. Anderson is Board certified in Podiatric Medicine. He graduated from Indiana University with a BS in Biology, and received his degree in Podiatric Medicine from New York College. Dr. Anderson was the Chief Podiatric Resident at Bergen Pines County Hospital in NJ, and is licensed in Massachusetts. He is a member of the American College of Foot and Ankle Surgeons as well as the Association of Sports Medicine. He has published numerous papers, including Cytomegalovirus Infection of BS-1, and worked on the FDA protocol for approval of Factor IX & BMP-2. Dr. Anderson has been a coach at the NBA Basketball Camp, and a Medical Volunteer at both the NYC and Boston Marathons. |

MARVIN B. CORLETTE, JR., MD, Surgery
Dr. Corlette is Board certified by the American Board of Surgery and is licensed to practice in Massachusetts. He graduated with honors from Stanford University, received his MD from Harvard Medical School, was a General Surgery Intern at UCLA Medical Center, a General Surgery Resident at Boston City Hospital, Harvard Service, a Hepato-biliary Surgery Fellow at the University of Paris, and has taught as an Assistant Clinical Professor of Surgery at Harvard Medical School. Dr. Corlette has held hospital appointments at the Lahey Clinic and Symmes Hospital where he was the Chief of Surgery. He maintains an active role teaching physical diagnosis to Harvard Medical School students, and to surgical residents who seek an understanding of his unique technical skills. He is a well respected contributor to major national and international publications, and is
currently developing an article with a Professor of Surgery in Torino, Italy.

SUSAN LIVINGSTON MSW, LICSW, Social Work
Is a clinician with experience in the full continuum of psychiatric services including emergency assessment, inpatient, residential, partial hospital, day treatment and intensive outpatient treatment settings. She is also a guest lecturer and clinical trainer in the areas of depression, suicide, and PTSD, among others. She obtained a Bachelor of Arts degree from Salve Regina College in 1975, a Masters of Social Work from the University of Connecticut in 1985, and advanced training in Dialectic Behavior Therapy, Critical Incident and Stress Management.

SARAH SULSKY, MS RD
Registered Dietician, Nutritionist, Registered Diabetic Educator
Graduated from Simmons College in 1982 with a Bachelor of Science in Foods and Nutrition, and performed a Dietetic Internship at Brigham and Women's Hospital. She was a Clinical dietician at Frisbie Memorial, (New Hampshire) and Good Samaritan (Oregon) Hospitals, developed recipes and menus for Dover Middle School, provided therapeutic and general nutrition counseling for all ages at Martin's Point Healthcare, and has overseen Outpatient nutrition counseling for phase 2 and phase 3 cardiac rehab as well as diabetes education.

JOSEPH ZIBRAK, MD, Pulmonologist
Dr. Zibrak is Board certified in Internal Medicine and Pulmonary Medicine. Dr. Zibrak also practices intensive care medicine as an active member of the medical staff of Beth Israel Deaconess Medical Center and is also an Assistant Professor of Medicine at Harvard Medical School. Dr. Zibrak graduated cum laude from Colgate University, interned and completed a medical residency at the University of Michigan, and completed a Pulmonary Fellowship at Boston University School of Medicine. His major research interests include Interstitial Lung Disease, with published articles in the NE Journal of Medicine and Experimental Lung Research. Dr Zibrak sees adult patients in pulmonary consultation year-round at the Wellfleet Health Center of OCHS by referral from their primary care provider at OCHS. Dr. Zibrak also has a home in Wellfleet. |
