Outer Cape Health Services operates one of two Healthy Connections outreach and enrollment sites in Massachusetts. Healthy Connections is also a one of three Massachusetts local coalitions for the Robert Wood Johnson Foundation Covering Kids and Families Initiative.

There are programs such as MassHealth and The Children’s Medical Security Plan for people with jobs and those without jobs, citizens and non-citizens, disabled adults, and especially children. Many people qualify, so you or your family may qualify NOW! It is easy to apply and we can help.

Healthy Connections' mission is to connect eligible uninsured clients with publicly funded health insurance and other social service programs, and to ensure that clients effectively utilize benefits to which they are entitled. Our goal is to provide a warm and welcoming atmosphere and make the process as seamless as possible.

All services at Healthy Connections are free and confidential.

Healthy Connections works in partnership with the communities, residents, and human service providers to improve public health on the Lower/Outer Cape. We have community healthcare coordinators available to work with residents of Harwich, Chatham, Brewster, Orleans, Eastham, Wellfleet, Truro, and Provincetown. Healthy Connections is a program of Outer Cape Health Services in cooperation with the Lower/Outer Cape Community Coalition. Because of its efforts to reach out to the underserved and remove barriers to care, Healthy Connections is instrumental in identifying emerging issues that affect access to health coverage and healthcare services.
