rick levine, outer cape health services RICK LEVINE, MD, Primary Care
Dr. Levine is Board Certified in Family Medicine. Dr. Levine graduated magna cum laude from Ursinus College in 1988, taught high school biology for three years prior to going to medical school and graduated from The Medical College of Pennsylvania in 1994. After completing a family medicine residency at the West Jersey Family Medicine Residency in 1997, Dr. Levine stayed on as faculty for 13 years. A New Jersey "Top Doc" in 2008, Dr. Levine is interested in all aspects of family medicine, from birth to grave, including well child visits, GLBT health, chronic disease management, and preventive medicine. Dr. Levine practices at OCHS in Wellfleet as well as Provincetown.

BARBARA K. PRAZAK, MD

Medical Director, Director of Clinical Quality
Dr. Prazak is Board Certified in Internal Medicine. She is a 1976 magna cum laude graduate of Boston College with a BS degree in Biology, obtained her Medical Degree from the University of Massachusetts Medical School, Worcester in 1980, and performed her residency at Mount Auburn Hospital, Cambridge from 1981-1983. She has been a resident of Wellfleet and a physician at Outer Cape Health for over two decades. Dr. Prazak practices at OCHS in both Wellfleet and Provincetown.

William Shay, MD, Primary Care

Director of the Travel ClinicDr. Shay is Board certified in Internal Medicine. A magna cum laude graduate  of St. Lawrence University with a BS in Chemistry, he obtained his MD from Albany College, interned at St. Vincent's Hospital, and performed his residency at Metropolitan Hospital, also in NY. He is a Clinical Assistant Professor of Medicine at New York Medical College, and is a member of the American College of Physicians. Dr Shay brings over 25 years experience as a physician with particular depth in providing primary care to people with HIV and those with more complicated medical conditions. He is committed to prevention and partnership with his patients. Dr. Shay joined the OCHS staff after coming from a large group practice next to St. Vincent's Hospital in NYC.  He has had a home in Wellfleet since 1993.

Dr. Thuraisamy is Board Certified in both Internal Medicine and Pediatrics. She is a Doctor of Osteopathic Medicine and received her degree from the University of Medicine and Dentistry of New Jersey – School of Osteopathic Medicine in 1993. She completed her dual residency in Internal Medicine and Pediatrics at St. Vincent's Hospital and Medical Center of New York in 1997, and received her BA in Chemistry from Mount Holyoke. She previously practiced in rural Alaska and as a locum's physician in CA, Washington and NY. She is now seeing children and adults.

janet whelan, outer cape health services JANET WHELAN, MD, Internal Medicine, Urgent Care
Dr. Whelan is Board eligible in Internal Medicine, and recently retired as a primary care physician but will be continuing to provide some urgent care coverage at OCHS. She is a 1965 graduate of Harvard University, Radcliffe College. She obtained her MD from SUNY Downstate Medical College in 1981, and performed her residency at the University of Medicine and Dentistry of New Jersey, Internal Medicine Residency, from 1981-1984. Dr Whelan is a life long resident of Provincetown.

matt abar, outer cape health services MATTHEW P. ABAR, PA-C, Physician Assistant
Matt Abar has been a Physician Assistant-Certified at Outer Cape Health Services since 1988, providing primary care to a complete spectrum of patients, from infants to seniors. A Massachusetts native, he moved to New York to attend college, then started his medical training while working on whale watch vessels in Provincetown over several summers. That job inspired his return to Cape Cod after he earned his Physician Assistant Certificate at Albany Medical College, Union University, in Albany, New York. He appreciates working in a Community Health center where he is able to address everything from everyday health concerns to complex medical issues such as HIV/AIDS. Matthew is an avid cyclist and sailor.

CAROL DANSKY, PA-C, Physician Assistant

Carol is a Board Certified in Family Medicine and Pediatrics. She received her PA-C and Master of Health Professions in 1988 from Northeastern University in Boston, MA, and her BA in Biology from the University of Rochester in NY. Ms. Dansky welcomes adult patients for primary and
urgent care.

o'rourke, outer cape health services ROBERT O'ROURKE, PA-C, Physician's Assistant
Robert brings over 30 years of urgent care and orthopedic expertise to OCHS. He received his Master's degree in Health Education from Cambridge College and his PA degree from Pennsylvania State University College of Medicine in 1977. Bob also earned a Sc.D. degree from Penn State in 2009. He served in the US Air Force Medical Corps, including service in Vietnam, as well as the US Coast Guard Medical Corps Reserve, retiring in 2009 with the rank of Commander. He is an Assistant Professor of Orthopedics at Tufts University School of Medicine, and a member of the American Academy of Sports Medicine. Bob believes in patient education and preventive medicine, particularly as it applies to sports medicine injuries. He sees adult patients for urgent care and outpatient orthopedic care year-round in Provincetown.
 

o'toole, outer cape health services MARIANNE O'TOOLE, Nurse Practitioner
Marianne O'Toole is a primary care nurse practitioner who brings special expertise in geriatric medicine to Outer Cape Health. She received her BS from Worcester State College and her Masters in Gerontological Nursing from Massachusetts General Hospital in Boston in 1991. Ms O'Toole's professional experience includes outpatient primary care, urology and short and long-term rehabilitation care. She considers patient empowerment her primary goal in the care of her patients, and feels strongly that the combination of nursing and medical education in her background provides her with a holistic appreciation of her patients and their individual health goals. Ms O'Toole welcomes adult patients of all ages, and provides primary and urgent medical care fulltime at the Provincetown health center as well as regularly at Seashore Point Nursing Home in Provincetown.
 

roderick, outer cape health services SUSAN FLYNN RODERICK, CFNP, CPNP, BSN, Senior Nurse Practitioner, Director of the Teen Clinic
Susan obtained a BS in Nursing from the University of Rhode Island in 1971, her Pediatric NP in 1976 and Family NP Certificate from Northeastern University in 1980. She was the Massachusetts League of Community Health Centers 2001 Employee of the Year, and has worked with many social service organizations, including Independence House, Provincetown Domestic Violence Intervention Office, Cape Cares, and Helping Our Women. She has provided health education classes to middle school and high school students of the lower and outer Cape. Susan started the Teen Health Clinic in 1992 to teach and empower young adults to act responsibly, and has worked at OCHS for over 3 decades.

SUSAN ROSENBERG, FNP, Nurse Practitioner
Susan graduated from SUNY Brockport in 1974 with a BA in Communications, from the University of New Mexico in 1983 with a Bachelor of Science in Nursing, and from the State University of New York Buffalo with a Master of Science in 1992. She is currently a licensed Family Nurse Practitioner, Registered Nurse, Certified Family Nurse Practitioner, Basic Life Support and in ACLS. She is an adjunct faculty member at SUNY Buffalo, and is a preceptor for NP students. She has worked in Primary care and research with HIV/AIDS patients, made house calls to the homebound elderly and has provided preventative care and management of complex ongoing medical problems.

 

andrews CHERYL ANDREWS, DMD, Dentistry
Dr. Andrews has practiced General Dentistry in Provincetown since 1986. She received her Bachelor's Degree from Bates College in Maine, and graduated at the top of her dental class from the University of Pennsylvania, School of Dental Medicine. Dr. Andrews completed a one-year post-doctoral program in the Department of Advanced Dental Education and also taught part-time at Harvard University School of Dental Medicine. Dr. Andrews earned her Fellowship from the Academy of General Dentistry in 1994. She has been a member of the American Dental Association, the Massachusetts Dental Society, the Academy of General Dentistry and the Cape Cod District Dental Society for over 24 years. She remains current regarding the latest innovations and research in the fields of General Dentistry, Oral Medicine and Implantology.
 

cooper, outer cape health services DONNA R. COOPER, MD, MS, Urgent Care
Dr. Cooper is Board Certified in Internal Medicine and provides part-time Urgent Care at OCHS. She graduated summa cum laude from Marquette University with a BS in Medical Technology before obtaining her MD from University of Massachusetts Medical School. She obtained her Health Policy and Management MS from the Harvard School of Public Health. Dr. Cooper worked at Provincetown Health Center early in the AIDS epidemic, and continued her work at the Fenway Community Health Center before joining the staff at Massachusetts General Hospital where she serves on the Urgent Care staff. Throughout her career she has worked in policy and practice to streamline services to patients seeking the highest quality care. She is a 25 year resident of Provincetown.

MARVIN B. CORLETTE, JR., MD, Surgery
Dr. Corlette is Board Certified by the American Board of Surgery and is licensed to practice in Massachusetts. He graduated with honors from Stanford University and received his MD from Harvard Medical School.  He was a General Surgery Intern at UCLA Medical Center; a General Surgery Resident at Boston City Hospital, Harvard Service; and a Hepato-biliary Surgery Fellow at the University of Paris.  Dr. Corlette was the Chief of Surgery at the Lahey Clinic and Symmes Hospital. He maintains an active role teaching physical diagnosis to Harvard Medical School students, and to surgical residents who seek an understanding of his unique technical skills. He is a well respected contributor to major national and international publications, and is currently developing an article with a Professor of Surgery in Torino, Italy.

Narcis obtained his Doctorate Degree in Dentistry from the University of Oradea, School of Medicine and Pharmacy, in Oradea, Romania in 2003. He received his Diploma in Biology and Chemistry at College H.H.C., Alba Iulia, Romania in 1996. Since 2004, he has served in various dental positions at Mid-Cape Dental, Mid Upper Cape Community Center, Outer Cape Dental and Outer Cape Health Services, Inc. His goal is to serve patients in a community healthcare setting.

CHRISTOPHER ROWLEY, MD, MPH, Infectious Disease and Director of HIV Medicine
Dr. Rowley is Board Certified in both Internal Medicine and Infectious Disease, and is licensed in Colorado, Botswana and Massachusetts. He obtained his BA from the University of Notre Dame, his MD from Brooklyn SUNY where he graduate magna cum laude, and his MPH from the Harvard School of Public Health. Dr. Rowley was an intern and resident in Internal Medicine at the University of Colorado, Denver.  He was a Clinical Fellow in Infectious Diseases at Beth Israel Deaconess Medical Center, and was a Research Fellow in Molecular Biology of HIV at the Harvard School of Public Health. He has been appointed a Clinical Fellow in Infectious Diseases and an Instructor in Medicine at Harvard Medical School. Dr. Rowley is an Attending Physician in the Division of Infectious Diseases at BIDMC, and has held multiple visiting appointments at the Infectious Diseases Care Clinic in Gaborone, Botswana.

 

sulsky, outer cape health services SARAH SULSKY, MS RD Registered Dietician, Nutritionist, Registered Diabetic Educator
Sarah Sulsky graduated from Simmons College in 1982 with a Bachelor of Science in Foods and Nutrition, and performed a Dietetic Internship at Brigham and Women's Hospital. She was a Clinical dietician at Frisbie Memorial Hospital, New Hampshire and Good Samaritan Hospital, Oregon.  She developed recipes and menus for Dover Middle School, provided therapeutic and general nutrition counseling for all ages at Martin's Point Healthcare, and has overseen outpatient nutrition counseling for phase 2 and phase 3 cardiac rehab as well as diabetes education.
 

amy wolf, outer cape health services JOHN TREEFUL, RDH. M.Ed., Dental Hygienist
John received a BA in Spanish and his M.Ed. at the University of Phoenix in Arizona. He received his Certificate in Dental Hygiene at Massachusetts College of Pharmacy and Health Sciences in Boston, MA. Before joining OCHS, John served Outer Cape residents as a hygienist at a Wellfleet dental facility. John has also been a Dental Hygienist at Martha's Vineyard Hospital since August of 2009 and is a year-round resident of Provincetown.

Amy Wolf, LICSW, Social Work, HIV Counseling

Amy graduated in 1989 with High Honors from Wesleyan University in Middletown, CT where she obtained a Bachelor of Arts degree and was awarded the Walkley Prize for excellence in Psychology.  She went on to Hunter College School of Social Work in NYC, where she graduated in 1994 with an MSW in Casework.  She has spent time studying in both Kenya and the Netherlands .  She has worked at Outer Cape Health Services since 2003 and works closely with the HIV services team.  She enjoys working collaboratively with colleagues in the clinic, as well as in the Outer Cape community.

 

JAMES ANDERSON, DPM, Podiatry
Dr. Anderson is Board certified. He graduated from Indiana University with a BS in Biology and received his degreee in Podiatric Medicine from New York College. Dr. Anderson was the Chief Podiatric Resident at Bergen Pines County Hospital in NJ. He is a member of the American College of Foot and Ankle Surgeons and the Association of Sports Medicine. Dr. Anderson has been a coach at the NBA Basketball Camp and a medical volunteer at both the NYC and Boston marathons.
